It is with mixed feelings that I announce that Lander Family Medicine will be closing in September 2020.
I have spent the last 18 years quelling unfounded rumors of my imminent retirement. However, the recent deaths of my mother and brother, as well as other immediate family needs, require me to shift my attention and finally step away from Lander Family Medicine.
I know that transition in your healthcare provider can be challenging, and I want to reassure you that Steve Platz, PA-C, and I are developing plans to support your ongoing needs. Steve will begin working for the Lander Medical Clinic in September, and is happy to continue providing your care. We are still working out the details of patient scheduling and clinic locations, but if you need to be seen, simply call our office 332-1551. I will continue to work on a limited basis, mostly telemedicine, through August. We will have more guidance for you closer to September, and we will update this website as plans solidify.
If you intend to continue seeing Steve, he will retain access to your electronic medical records. If you would like to change providers, please let us know the name and contact information of your new provider by August 15, so we can forward your records. You may access your records on this Portal, or pick up a copy of your records from our office before September 1, 2020. All your records will be available at Lander Medical Clinic, beginning in September.
I will continue to serve as medical director of Help for Health Hospice, and have no plans to leave Lander. I have 29 years of wonderful memories serving the people of Fremont County. Thank you; it has been an honor to work with you.
